What we need from you

  • Full names and identity numbers of the people named on the record
  • A copy of the applicant’s South African ID or passport
  • Signed instruction and contact details for collection or courier
  • Deceased’s full names, identity number, and date of death
  • Your relationship to the deceased and reason for the request
