Process

What we need from you

  • Full names and identity numbers of the people named on the record
  • A copy of the applicant’s South African ID or passport
  • Signed instruction and contact details for collection or courier
  • Child’s full names, date and place of birth
  • Parents’ full names and identity numbers where known
